AOX In-Office Plan

Many dental insurance plans have limitations, such as requiring group enrollment, high costs for individual plans, and restrictions, exclusions, or high co-pays for more affordable options. Furthermore, after a full-mouth restoration, many providers classify subsequent check-ups as cosmetic and cease payments.

To ensure our AOX patients receive high-quality, affordable care, we've developed an exclusive in-office dental plan. An AOX patient check-up is priced at $517 annually without insurance.

Terms and Conditions:

  • Non-Refundable Fee: The annual pre-paid fee is non-refundable.
  • Non-Transferable: This plan is exclusively for the enrolled individual.
  • Service Completion: All services covered must be completed within one year.
  • Fee Adjustments: Reduced fees are subject to an annual increase to account for inflation and cost of living.
  • Patient Responsibility: Patients are responsible for scheduling and receiving all covered benefits, including exams and cleanings.
  • Appointments & Cancellations: Appointments are required. We must be notified of cancellations at least 48 hours in advance.
  • No Other Discounts: This plan cannot be combined with any other discounts.
  • Payment Due: Payment is due at the time of service unless prior financial arrangements have been made.

Special offers for Gift Cards and Teeth Whitening require new patients to complete an appointment. Offers cannot be combined, are limited to one per patient, and must be used within one year of your first appointment. If you’re a new patient without insurance, you may qualify for free exams and X-rays. Please note, additional appointments may be necessary for optimal dental care. We have a cancellation policy in place requiring 48-hours notice for regular appointments and 4 business-days for specialists. Non-compliance incurs a $50 fee for standard appointments and $150 for specialty appointments.
